Data Collection, Collation, Analysis, Storage and Dissemination<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">3. Develop Key Performance Indicators for the State Primary Health Care System<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">4. Implementation of Health Insurance policies, guidelines and other Financial Risk Protection and Health Care Financing Policies strategies<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">5. Production, dissemination and Coordination of HMIS data tools in the PHCs<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">6. Conduct Data Quality Assurance (DQA)<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">7. Conduct Need assessment of Infrastructure and Equipment and coordinate appropriate interventions<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">8. Providing and managing SPHCDA website<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">9. Conduct Baseline, midline and End line Studies and Operational Research<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">10. Develop and implement HRH capacity building plan<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">11. Develop and Implement training programmes for health workers<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">12. Develop and manage data base for PHC system<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">13. Coordinate PHC related Development Partners activities.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">14. Track the Implementation of Strategic and annual operational plans and facilitate periodic review of such plans<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">15. Ensure effective 2 way referral system by exploiting all available means e.g Emergency Transport Scheme.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">16. To ensure provision of PHC laboratory equipment and services in conjunction with the relevant departments.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">17. Create and keep record of all relevant PHC related policies and plans in the State<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">18. Coordinate the function of LGA\/Zonal PHC Coordinators in conjunction with other departments<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">19.
